A complete course of transference-focused psychotherapy—extended (TFP-E) will have an opening phase, a middle phase, and an advanced phase with an ending, or termination. In this chapter, we discuss the three phases of TFP-E, focusing on the central tasks of each treatment phase. We consider the impact of level of personality organization on the overall trajectory of treatment and on the clinical issues that characterize each phase of treatment. In addition, we introduce markers of change that signal the patient’s readiness for the next phase of treatment.
