In this chapter, we introduce the basic therapeutic tasks of transference-focused psychotherapy—extended (TFP-E), both to provide an overview of the treatment approach and to introduce constructs that will be discussed in detail in the chapters that follow. We define key terms and familiarize the reader with the basic elements that form the conceptual foundation and the technical building blocks of the treatment. Our overall approach is to focus on promoting the coalescence of conflictual object relations to foster identity consolidation and integration, which can take place at increasing levels of complexity. As we explain, integrative processes are different at different levels of personality organization. At the same time, there are unifying objectives and clinical principles that organize work across the spectrum of severity. In this chapter, we review basic elements of the treatment while calling attention to how these elements may serve different functions or be subject to differential implementation as a function of severity of personality pathology.
